L’app Diario Arnica è una sperimentazione della ATS Montagna, rivolta ai pazienti cronici aderenti a un percorso di presa in carico, che hanno accettato il protocollo di sperimentazione presso il proprio gestore.
L’app permette di:
• compilare il questionario di autovalutazione per l’assunzione dei farmaci e condividerlo tramite il Fascicolo Sanitario Elettronico (FSE);
• consultare il Piano di Assistenza Individuale (PAI);
• inserire nel taccuino personale dati e documenti relativi ai percorsi di cura, anche effettuati presso strutture al di fuori del SSN e condividerli tramite FSE.
Per l’utilizzo dell’app è necessario aver:
• aderito al nuovo percorso di presa in carico di Regione Lombardia;
• accettato la sperimentazione Arnica;
• espresso il consenso all’alimentazione e consultazione del FSE e aver dato l’autorizzazione al pregresso.
